we have had the same problem from time to time now playing version 8.67d.
first of all we dont get any leaver gold for the person out.
second there was one game where the person was kicked for feeding and the tank remained in the game until killed by our team. after that no respawn.
rest was named by prog (person still listed as being in the game, etc.)
